Whether you are coming to our office for your appointment or mailing your information to us, the following is a checklist of items we need in order to prepare your income tax return.
- All W-2, 1099, and 1098 forms
- Any closing papers from the sale or refinance of your home or vacation property
- All year-end statements for mutual funds you held
- Your prior two tax returns, if TaxPrep Solutions, Inc. did not prepare your last year's return
- Medical expense records, including unreimbursed bills for doctors, dentists, chiropractors, therapists, prescription eyeglasses and contact lenses, medical insurance, long-term care insurance, prescription drugs, and medically-related transportation costs
- Other tax records including your real estate tax statements, vehicle license tab receipts, records of any state income taxes paid, and records of any estimated tax payments
- Interest expense records including your mortgage interest (Form 1098), home equity loans or lines of credit and any contracts for deeds you have on your homes, including the name, address, and social security number of the person to whom you are making payments
- All contribution records - both cash/check and noncash
- Records relating to miscellaneous deductions, including tax preparation fees, union dues, employee expenses not reimbursed by your employer, and certain investment expenses
- If you moved within the past year, any moving expense records, including the cost of transportation of household goods and personal effects and travel (including lodging by not meals) to your new residence (to be eligible for the moving expense you must meet a distance test, a length-of-employment test and a commencement-of-work test.)
- Records of tuition and course fees paid for higher education for you, your spouse, and your dependant children
- Records of interest paid for student loans
- Receipts for allowable education (K-12) expenses, including tuition, school supplies and other school related items inorder to claim education credits on your Minnesota Tax Return
- Records relating to childcare costs, including the name, address, and identification number (SSN if an individual provided the care) of the provider
